Step 1: Enter Receipt Information

Please follow the steps below to enter your initial purchase receipt information. Please enter all of your receipts from your initial purchase of the brake hose/brake kit before moving on to the registration page. You may submit up to 5 receipts. If you have more than 5 receipts, please call the recall hotline at the top of this page and they will be able to assist you.

Please note, this is the receipt for the initial purchase of the brake hose/brake kit. Please do not upload any receipt related to removal of the affected hoses. Instructions for submitting removal receipts will be included in your approval email.
